Managing Beyond Your Walls: How the FY 2027 IPPS Proposed Rule Changes What Hospitals and ACOs Must Be Able to Do

CMS is proposing a mandatory nationwide episode-based payment model that extends accountability 90 days beyond discharge. The organizations that build the right infrastructure now will be the ones best positioned to lead.

Reading the Signal from Medicare

Each year, healthcare leaders review the CMS Inpatient Prospective Payment System rule for changes affecting hospital payment, coding, quality programs, and operations. That annual review is necessary — but it can also become too narrow.

When organizations look at each provision on its own, they may miss the larger direction Medicare is signaling. The FY 2027 IPPS Proposed Rule offers an important example.

What Is CJR-X?

Among its key provisions, CMS proposes a new model called the Comprehensive Care for Joint Replacement Expanded Model — CJR-X. If finalized, CJR-X would become a mandatory nationwide episode-based payment model beginning October 1, 2027. It would cover lower-extremity joint replacements with a 90-day post-discharge window.

When considered alongside the mandatory Transforming Episode Accountability Model (TEAM), which began January 1, 2026, CJR-X signals that Medicare is moving hospitals from responsibility for a single encounter toward accountability for cost, quality, and coordination across a full episode of care. This is more than a new payment method. It represents a different hospital operating model.

Episode Accountability Is Moving Beyond Optional

Voluntary and mandatory models create different expectations. Voluntary models tend to attract hospitals that already have the tools, physician alignment, and analytics needed to manage risk. Mandatory models bring a much wider range of organizations into the shift — including hospitals that may not have chosen to take on episode-level accountability on their own.

TEAM is already running as a mandatory episode-based payment model in selected markets. It covers five surgical episode types — including joint replacement, spinal fusion, bypass surgery, and major bowel procedures — with episodes extending 30 days after discharge. CJR-X would go further, requiring most IPPS hospitals to take part nationwide.

Hospital leaders should stop treating bundled payment as a finance project that only activates when a hospital chooses to opt in. The skills needed to succeed under these models are becoming core operating requirements.

The Hospital Stay Is No Longer the Full Unit of Performance

Under CJR-X, an episode begins with a qualifying joint replacement and runs through the first 90 days of recovery after discharge. CMS would compare actual episode spending with each hospital's target price.

Depending on spending and quality results, the hospital could receive a payment or be required to repay Medicare. A clean procedure and smooth hospital stay may no longer be enough.

What Can Go Wrong After Discharge

A patient may still face delayed therapy, an avoidable ER visit, a drug-related issue, an unneeded skilled nursing stay, or a preventable readmission — all of which can affect episode results even after the patient leaves the building. Episode accountability is not just an extension of the hospital stay. It requires organizations to see the recovery period as a connected clinical and financial whole.

Post-Acute Care Is Becoming Hospital Strategy

A hospital may not own the skilled nursing facility, home health agency, rehab provider, or physician practice handling the next phase of care. Even so, the cost, quality, timing, and coordination of those services can affect the hospital's episode results.

Post-acute relationships can no longer be treated only as discharge planning tools — they are becoming part of core strategy. Organizations need to know which providers get strong results, where communication breaks down, whether services are available on time, and which post-acute patterns lead to avoidable readmissions.

Five Questions Every Organization Should Be Able to Answer

  • Do we know the full cost of the episode? Including post-acute care, therapy, follow-up services, ER use, and readmissions.
  • Can we find the source of variation? Separating patient-driven, clinically needed, geographic, provider-related, and process-related causes.
  • Do service lines own downstream results? Responsibility should not end when the patient leaves the operating room.
  • Do we know our post-acute environment? Visibility into provider results, communication reliability, and patient access barriers.
  • Can we explain our results clinically? Financial and usage data must be read alongside accurate clinical information.

How Ensure Data Solutions Supports Episode-Based Performance

The tools needed to succeed under CJR-X and TEAM are the same ones the Ensure platform was built to deliver — connecting clinical data, network intelligence, care management, risk adjustment, and financial analytics into one unified system.

Network Intelligence & Referral Optimization

EnsureXcel© — High-Performance Network Intelligence ↗

As post-acute results become part of hospital strategy, organizations need data to identify which providers get strong outcomes, keep costs in check, and coordinate well across settings. EnsureXcel© turns real-world performance data into a clear, actionable network strategy.

  • Provider scoring across outcomes, total cost, episode efficiency, readmissions, and guideline follow-through
  • Referral pattern tools — surfacing preferred post-acute and specialist providers and flagging high-cost gaps in real time
  • Network setup by specialty, geography, payer contract, and patient group
  • PCP performance data to support ACO growth and network expansion
  • AI-driven engine that updates provider scores using real-world evidence
Care Management

EnsureCM© — Advanced Care Management ↗

Managing episode results across 30 or 90 days requires active, real-time care management that goes beyond the hospital walls. EnsureCM© alerts providers to key clinical events — including admissions, readmissions, new diagnosis codes, and ER visits.

In addition, it supports patient engagement through FHIR connectivity, remote patient monitoring, and care coordination across home, acute, sub-acute, and outpatient settings.

  • Real-time alerts for admissions, readmissions, and ER visits via omnichannel notifications
  • Predictive risk grouping to find patients most at risk of avoidable post-discharge issues
  • Care plan tools supporting smooth moves between care settings
  • Remote patient monitoring to track outcomes after discharge
  • FHIR connectivity for patient engagement through EMR care events
Risk Adjustment & Clinical Documentation

EnsureRAF© — Risk Adjustment Management ↗

In an episode-based setting, clinical documentation helps explain results clearly. It may show whether resource use reflected real patient needs, an unexpected event, or an avoidable gap in care. EnsureRAF© supports forward-looking and backward-looking condition tracking, documented and undocumented condition review, risk score tools, and AI-powered documentation review.

Financial & Utilization Analytics

EnsureFin© — Financial & Utilization Analytics ↗

Understanding the full cost of an episode — including post-acute care, therapy, follow-up, and readmissions — requires financial visibility that goes far beyond the inpatient claim. EnsureFin© delivers cost and usage trend data, admissions per 1,000, readmission tracking, ER visit analytics, peer benchmarking, and provider stop-loss analysis.

Interoperability

EnsureXchange© — Interoperability ↗

A late discharge summary, an incomplete medication list, or an unclear follow-up plan can create risks across the full episode. EnsureXchange© enables smooth, two-way exchange of clinical data between EHR systems and the Ensure platform — supporting FHIR connectivity, point-of-care alerts, chart retrieval, ADT-driven hospital management, and structured data exchange across providers, payers, and post-acute settings.

The Path Forward

Medicare is moving from single services toward connected episodes. It is shifting from responsibility for the hospital visit toward responsibility for recovery. It is moving from optional pilots toward wider financial accountability.

The key question is no longer what changed in the annual rule. The more useful question is what kind of organization Medicare is asking hospitals and ACOs to become. The signals point toward organizations that manage beyond their walls, explain variation clearly, build strong post-acute relationships, and accept accountability for both results and resources.

Ensure Data Solutions was built to support exactly that shift — helping hospitals, ACOs, MSOs, and health systems build the data foundation, network intelligence, and care coordination tools that episode-based accountability demands.
