Star Ratings Measure Set Shrinks: What the CY2027 Final Rule Means for Cut-Point Strategy

CMS has removed 11 administrative measures from the Star Ratings program. As the measure set shrinks, the remaining clinical and experience measures carry more weight — and every point gained or lost matters more than ever.

What Changed in the CY2027 Final Rule

The CY2027 Star Ratings final rule removes 11 admin measures from the Medicare Advantage Star Ratings program. These include call center metrics, appeals timing measures, and other process-based measures that many plans had relied on as steady points year after year. As a result, the total measure set is now smaller — and the measures that remain carry a lot more weight.

Specifically, clinical measures are expected to make up about 65% of the overall Star Rating under the new setup, up from roughly 50% in 2025 and 2026. In addition, the drop in patient experience, complaints, and access measures from a weight of 4 to a weight of 2 was finalized in the CY2025 rule and took effect with the 2026 Star Ratings. Furthermore, a new Part C Depression Screening and Follow-Up measure is set to begin in measurement year 2027 and be included in the 2029 Star Ratings.

What This Means for Cut-Point Strategy

The removal of admin measures changes the math behind Star Ratings in two key ways. First, it removes a floor that many plans counted on. Admin measures — call center metrics, appeals timing, and similar process-based items — responded well to good operations and were reliable points for plans that managed them well. Losing them removes a steady source of performance.

Second, it puts more pressure on clinical and experience measures. When 43 measures carry a rating, one weak area gets spread across the full set. When the set shrinks, a single weight-3 outcome measure moving half a star matters more than it used to. Moreover, Star Ratings work like a tournament — a plan's score is graded on a curve set by the whole industry. Consequently, if the industry improves faster than your plan does, the rating can fall even as raw performance goes up.

The strategy that worked five years ago no longer applies. Old thinking: improve HEDIS rates and stars go up. New reality: improve HEDIS rates, but so does everyone else — and cut points may rise faster than your gains. Every plan needs to rework its weighted measure set under the new, leaner structure.

The Timing Window Plans Should Not Miss

There is a real opportunity in the timing. The first measure removals take effect with measurement year 2026, which is already in progress. However, the biggest changes arrive with measurement year 2027 — which has not yet started. As a result, plans that act now have a full planning cycle to rework their weighted measure set, find where effort still moves ratings, and test paths to the next star level.

In other words, the window is open — but it will not stay open. Plans that wait until the new measure set is fully in place will be reacting rather than planning. Furthermore, because Star Ratings run on a multi-year timeline, the steps taken today shape the ratings and bonuses received two to three years from now.

Care Management

EnsureCM© — Advanced Care Management ↗

Year-round care management is now essential to Star Ratings results — not just a nice-to-have. Consequently, EnsureCM© uses predictive tools to group patient groups by risk, trigger timely clinical steps, and support proactive patient outreach. In addition, it connects directly with EMR systems through FHIR, enabling care teams to act on care gap data at the point of care.

The Path Forward

The CY2027 final rule is not simply a policy update. It is a structural change in how Medicare Advantage Star Ratings are calculated — and in what kind of work earns points. For health plans that built their approach around admin measure results, the path forward calls for a fresh start. For plans that already focus on clinical outcomes and experience measures, it is a chance to pull ahead.

In either case, the organizations that succeed will be those that rework their weighted measure set now, find where effort still moves results under the leaner setup, and invest in the data tools and care management skills needed to keep clinical results strong year after year. Ensure Data Solutions was built to support exactly that work.
